Let's Talk about Mental Health
Let's Talk about Mental Health

Connecting Communities.
Removing Barriers.
Strengthening Behavioral Health.

The Talk Together Initiative is a community-based outreach program designed to connect individuals and families with behavioral health services while addressing the barriers that often prevent access to care. Through collaboration, outreach, and relationship-building, Talk Together brings providers, community organizations, and behavioral health teams together to create a more connected and supportive network of care.

What is Talk Together?

Talk Together is built on the belief that access to behavioral health support should be simple, welcoming, and community-centered. We work directly within communities to connect individuals with resources and services that support mental wellness and overall well-being.

Our initiative helps reduce barriers such as:

  • Transportation challenges

  • Lack of childcare

  • Limited awareness of available services

  • Difficulty navigating systems of care

  • Social isolation and disconnected support systems

By meeting people where they are, Talk Together creates pathways to care while fostering meaningful relationships among providers, behavioral health teams, and community partners.

How We Help

Community Outreach & Navigation

Our outreach teams engage directly with community members to identify needs and connect individuals to appropriate behavioral health and social support services.

Provider Collaboration

Talk Together strengthens communication and partnerships among behavioral health providers, healthcare systems, schools, nonprofits, and community organizations.

Barrier Reduction Support

We help address practical barriers that impact access to care, including transportation coordination and childcare support when available.

Community Connection

We create opportunities for individuals, families, and providers to build supportive relationships that promote long-term wellness and resilience.

You Don’t Have to Navigate Mental Health Alone

Let’s Talk. Let’s Connect. Let’s Build Together.

Whether you are seeking support, looking to partner, or wanting to strengthen behavioral health resources in your community, Talk Together is here to help.

Why Collaboration Matters

Behavioral health challenges impact entire communities — and meaningful solutions require collective action.

Talk Together creates a collaborative network where:

  • Providers can coordinate services more effectively

  • Organizations can share resources and referrals

  • Community members can access support more easily

  • Behavioral health teams can build stronger community trust

Together, we create a more connected system of care.
